Pay Deposit

To complete your scheduling request, we require a $50 deposit to secure your selected appointment time. This deposit will be credited towards your New Patient Exam in our office. If you need to reschedule your appointment after booking, you may do so at no additional cost; by simply calling our office at 630-868-8480, and giving at least 24 hours notice. This deposit is non-refundable if you later choose to not come to our office for care. Click the button below to start the payment process.
